Contemporary red silk on wooden boards, with arms of Heinrich Surbrick worked in silver and gold thread over raised backgrounds, shaded with blue thread, surrounded by the embroidered motto "Hinrich Surbick me fit faire", dated Paris, January 1626, in silver and gold thread, the back cover with similar raised work, embroidered in metal thread in a stylised foliate cruciform design, spine in three compartments with scroll designs in gold thread, the whole semé with sequins.
